Cognitive-Behavioral Therapy (CBT)

Cognitive-behavioral therapy, or CBT, is a form of psychotherapy or treatment for mental illness. It comes in a variety of methods, but the basic concept behind all CBT is the same -- thoughts cause feelings, which cause actions. If someone wishes to change problematic behaviors or emotions in their lives, they need to start by changing their thoughts. CBT examines ideas and looks for patterns that may be causing harmful actions. The therapist helps patients modify those thought patterns and, in doing so, helps them feel better and cope more effectively.

CBT is one of the most widely studied forms of psychotherapy, and it has been shown to be extremely effective for a variety of mental illnesses. Some of the issues that respond well to CBT include mood disorders, personality disorders, eating disorders, substance abuse, sleep disorders, and psychotic disorders. In some cases, CBT has been shown to be as effective or even more effective than medication. One of the interesting things that the scientific study of CBT has shown is that CBT actually changes the way the brain works, physically improving its function.

CBT differs from traditional psychotherapy in a few key ways. One of the most important distinctions is the emphasis on the power and responsibility of the patient in CBT. The patient will be encouraged to be the one asking the questions in CBT therapy, and most patients are assigned homework to complete outside of therapy sessions. There is a concept in CBT that everyone has power the power to change how they feel, even if they cannot control the situation, and this can be very empowering for patients. Because of this power shift, the therapist-client relationship is not as critical to success in CBT as it is in other modes of therapy. Patients should still get along well with their therapists, but they do not need a deep, dependent emotional connection to them. Finally, because CBT often treats a specific issue or problem, it is usually shorter in duration than traditional therapy. While some therapies may continue for years, CBT lasts on average just 16 sessions.

Deep Brain Stimulation (DBS)

Deep brain stimulation (DBS) is a procedure performed to help treat neurological conditions such as Parkinson's disease and epilepsy. Electrode leads are surgically implanted in the brain and connected to a device, called a neurostimulator (deep brain stimulator), that is placed underneath the skin (subcutaneously). The deep brain stimulator can send electrical signals through the electrodes to the brain to restore normal rhythms, or it can block electrical signals in specific areas of the brain to restore functioning. Although not a cure, DBS can treat symptoms of movement disorders such as pain and tremor when medication fails to work.

DBS surgery may be completed in one operating session, but it is more commonly performed in two parts. In the first stage of the procedure, the surgeon will map targets in the brain using CT or MRI imaging to determine where to place the electrodes. Then, by drilling holes into the skull, the surgeon will place the electrodes into specific areas in the brain. The holes will be closed, and patients will require 1-2 days to recover in the hospital. After about two weeks, patients will undergo another surgery to have the deep brain stimulator implanted. The stimulator is usually placed under the skin around the collarbone, chest, or abdomen, and it will be connected to the DBS electrodes through a subcutaneous wire. No component of the DBS system will be visible from outside. Following stimulator implantation, patients may need to recover in the hospital for 1-2 days.

Within 2-4 weeks of the operation, doctors will program the stimulator using a wireless device. Patients will work with their doctor to determine the most effective settings for the stimulator during follow-up visits. Patients will be given a handheld device so that they can turn the stimulator on or off and adjust its settings themselves. The stimulator may have a rechargeable battery, in which case patients will be given a charging unit. Stimulator batteries generally need to be replaced in 3-5 years.

Spinal Cord Stimulation (SCS)

If conservative treatment options and other spine surgeries have failed to provide someone pain relief, they may be a candidate for spinal cord stimulation. Spinal cord stimulation is a treatment for chronic back or leg pain. A spinal cord stimulator, which is surgically implanted underneath the skin (subcutaneously), can deliver electrical pulses to the area of pain. These pulses will reach the brain before the pain signals can, so instead of pain, patients will experience a tingling sensation.

To determine if spinal cord stimulation may benefit someone, they will first undergo a trial period. During this period, a temporary electrode lead is placed subcutaneously above the spinal cord, and it is connected to an external stimulator that they will carry with them. The procedure is typically completed within an hour, and patients may return home the same day. After a few days or weeks, the trial stimulator will be assessed for effectiveness. If the trial is unsuccessful, patients may undergo a second trial period, or the leads will be removed, and other treatment options will be considered. If, instead, a patient's pain is relieved during the trial, they will undergo surgery for implantation of a permanent stimulator and leads.

The components of a spinal cord stimulation system include the stimulator, the leads, and the wire that connects the two. Implantation of permanent leads may be percutaneous (through the skin) or involve a surgical incision. The spinal cord stimulator is implanted subcutaneously in the abdomen or buttock. The stimulator is battery-powered and may be rechargeable. If the battery is not rechargeable, it will last around 2-5 years, and a patient will need surgery to replace it.Their doctor will program the neurostimulator following the procedure.

After spinal cord stimulator surgery, patients may be released from the hospital the same day or the next one. In the weeks following the operation, patients will need to work with their doctors to find the optimal settings for their neurostimulator. Activities such as driving, twisting, bending, raising one's arms, sleeping on one's stomach, or lifting heavy objects should be limited.
