Anemia

Anemia is the most common blood disorder. It occurs when the body does not have enough red blood cells to carry oxygen to the tissues. Blood loss, insufficient red blood cell production, and red blood cell destruction are the three main causes of anemia. These issues may arise from such conditions as iron deficiency, heavy periods, ulcers, kidney disease, as well as some forms of cancer.

An individual with anemia will have the following symptoms: shortness of breath, weakness, dizziness, cold hands or feet, paleness, and irritability. Anemia is diagnosed when blood tests show low counts for hemoglobin, which is the protein responsible for carrying oxygen to different tissues throughout the body.

Treatments for anemia are dependent on the kind of anemia an individual has. For example, iron supplementation may correct iron deficiency anemia. For anemia caused by chronic kidney failure, chemotherapy treatments, and HIV/AIDS, a group of medications called erythropoiesis-stimulating agents may be prescribed.

Musculoskeletal Problems

The musculoskeletal system refers collectively to the bones, joints, ligaments, muscles, and nerves. It is what gives the body structure and movement. Because this system encompasses so much of the body, musculoskeletal problems are extremely varied and can happen almost anywhere. There are musculoskeletal problems that affect only the joints, those that affect the bones, those that affect tendons and ligaments, and those that can happen in any area of the body but cause pain and numbness.

Joint problems include bursitis and arthritis. Bursitis is the inflammation of a fluid-filled sac cushion on the outside of a joint, causing symptoms including pain and swelling. Arthritis is the inflammation and damage of a joint due to wear and tear or disease. There are several treatments for arthritis, but if the damage progresses far enough, joint replacement may be necessary. In joint replacement, a metal or plastic implant is surgically placed within the joint to make movement easier.

Bone problems include scoliosis, fractures, and osteoporosis. Scoliosis is a curvature of the spine, making the normally straight spine look like a 'C' or 'S.' It is treated with braces or surgery. Osteoporosis is a disease that causes bones to weaken and break easily. It's most common in women, especially older women. Fully half of all women over 65 have osteoporosis. Fractures are any break in the bone. They can be a simple crack or a severe and complicated shatter. Osteoporosis causes fractures in people who have it, but other causes are trauma (such as a fall) or overuse.

Tendon and ligament problems most often result from injury or overuse. Two good examples are ligament tears and tendonitis. The most common ligament tear is the ACL tear in the knee. This ligament supports and stabilizes the knee and is most often torn during sports activities. Tendonitis happens when a tendon, which connects muscles and bones together, becomes irritated and inflamed. This happens most often in older patients who push their bodies too far, leading to pain and swelling. Treatment for both tendon and ligament problems usually includes rest, ice, and supporting the area to let it heal.

Pain and numbness problems can be caused by overuse, disease, an injury, or a compressed nerve. Lower back pain and carpal tunnel syndrome are two of the most common conditions patients encounter. Lower back pain may be caused by sore muscles that have been overworked, or an injury to the disks separating the vertebrae in the spine. Lower back pain usually goes away on its own within a few days, but it may require medical treatment depending on the cause. Carpal tunnel syndrome occurs when the nerve at the base of the palm becomes trapped or pinched. Symptoms include tingling, pain, numbness, or weakness in the hand. Treatment involves rest and sometimes steroid injections.

Because the musculoskeletal system involves so much of the body, problems here can be incredibly varied in their type and severity. Some musculoskeletal problems will go away on their own or only require rest, while others may require medication, physical therapy, or even surgery.

Thrombocytopenia (Low Platelet Count)

Thrombocytopenia is a disorder where people have too few platelets. With too few platelets in the bloodstream, the blood is unable to properly clot. People with thrombocytopenia may experience prolonged external and internal bleeding. Large purple bruises (ecchymoses) and small red skin spots (petechiae) in the skin and mouth are common in those with thrombocytopenia. Other symptoms of the disorder include bleeding gums, blood in bodily fluids, and fatigue. Thrombocytopenia most often arises from a different condition or treatment, such as alcoholism, autoimmune disease, cancer treatments, viruses (such as Hepatitis C and HIV), bone marrow diseases (such as leukemia and aplastic anemia), enlarged spleen, and exposure to toxic chemicals. In rare cases, thrombocytopenia is inherited.

If a doctor suspects a patient has a thrombocytopenia, a complete blood count (CBC) or platelet count test helps determine the concentration of platelets in a person's blood at a given time. Thrombocytopenia can be diagnosed by hematologists using advanced tests on platelet samples. Flow cytometry tests detect abnormalities by evaluating the characteristics of a platelet sample using targeted lasers and lights.

Treatments for low platelet counts include steroids like Prednisone, that can assist with platelet production. If a patient's low platelet count is caused by spleen problems, spleen removal (splenectomy) may be recommended. To treat the prolonged bleeding caused by thrombocytopenia, doctors frequently prescribe Dempressin, a hormone that promotes blood clotting. Antifibrinolytic medications such as tranexamic acid may also aid in the promotion of clotting and the prevention of bleeding. For patients with severe platelet disorders (either low platelet count and dysfunctional platelets), blood transfusions may be prescribed. Platelet blood transfusions deliver donated platelets to patients intravenously, replenishing the volume of functioning platelets.

Patients can also modify their lifestyle to better manage their disorder and minimize complications. Patients with low blood counts should avoid blood-thinning medications, alcohol, and activities that could lead to injury. Many people with thrombocytopenia can live healthy and fulfilling lives with proper treatment and precautions.

Dr Tsai spent a lot of time answering my questions and explaining test results and course of treatment. She does this making sure I understand without talking down to me. She speaks directly and honestly but in a calm and relaxed manner. I find this reassuring when evaluating my cancer and course of treatment. I have the utmost confidence in her.She is well informed of other Swedish services that do not relate to my visit. She directed me to the pharmacy in First Hill building for walk in Covid and RSV vaccine. Previously a Swedish neurologist suggested I try acupuncture but could give no recommendations of where to go. I have also asked my primary doctor about Integrative medicine at Swedish and told there was no one practicing at Swedish. Dr Tsai directed me to Swedish Pain Clinic. This was the information I needed 3 years ago and no other Swedish doctors I saw were aware of this. As a result I have received acupuncture elsewhere and the Integrative medicine appointment I've waited for at UW Is later this month.I am bracing myself for an unpleasant treatment but look forward to seeing Dr Tsai again.

I am so very blessed to have Dr. Tsai leading my treatment team. She monitors my care and tests very closely and makes very informed decisions based on the tests and how I'm doing. We're starting a new cadence of IV Infusions based on the aforementioned, which was preceded by a Tx break since I was so exhausted and my body was so very tired.The break has made a tremendous difference in my physical, mental, and spiritual health. I so needed this break and am so grateful I have been stable for so long to allow this break. She is absolutely brilliant, exceedingly compassionate, super supportive, a fantastic listener, and a true asset to SCI True Center (pun intended). She deserves a big raise!!

Dr. Tsai is an incredible doctor who sets the standard for how other doctors should run their practice and patient care. She treats each patient with respect and acknowledges the unique needs each of us has. I appreciate how she is able to distill complex health issues into a treatment plan that supports the patient, and the challenges they face in an insurance led industry that often puts profit over appropriate patient centered healthcare. I wanted to give this feedback after my surgery 2 years ago and don't believe I ever did. I am so impressed with Dr. Tsai and her team. She is always willing to listen and to help find a solution to my issues(s). During my surgery, she held my hand as the team prepped me. I've NEVER had a doctor do that. Generally they aren't even in the room when the team is prepping. Then when I came to from the surgery, one of her staff was handing my hand while they were unhooking me from all the equipment, etc. That level of care and concern touched my heart. Thank you!!!!

I enjoy speaking to Dr. Tsai. She seems to remember everything I've said in the last four months. She Addresses any concerns politely and in a way that I can understand without being dismissive or disrespectful. My only bad experience isn't with Dr. Tsai directly, but with the wait time for medications at the infusion center. I waited nearly an hour for the drugs to arrive, making my anxiety increase.

Dr. Tsai is my oncologist that I see every six months. We discuss the affects of the prescribed treatment. I had a constant cough, which I thought was due to the medication. She recommended that I get chest x-rays. As a result I had an infection and she prescribed antibiotics, which has cleared my coughing. I am so grateful to Dr. Tsai! Thank you so much for your care.

The only thing was difficult is that the Doctor speaks very softly and I had to have her repeat her comments which she did very willingly. This was my first visit with Dr. Tsai as she was recommended to by current Doctor who retiring. I was very pleased with his joice for me and I am looking forward to working with Dr. Tsai in the continuing care of my treatments.

Dr. Tsai actually came to the infusion center to see me as the blood draw team schedule conflicted with my appointment with her.It turns out the blood draw team wasn't called until I check in and had been seated at the infusion center.She is absolutely fantastic in addition to being brilliant, compassionate, and supremely supportive.She deserves a raise!
